Today's Stampin' Pretty 1 Minute to WOW! Video Tutorial demonstrates the simple scores and folds involved.  Card stock was cut to 11 x 4 1/4" and scored with the Stampin' Up! Simply Scored Scoring Tool the short distance at 2 3/4" and 5 1/2".

I think these pictures do a good job of sharing the "reveal" for a recipient from start to finish!
